MEDICATION SAFETY TALKING POINTS

  • A great deal of complications after discharge from the hospital are due to medication errors.
  • It is also a huge contributing factor to hospital readmission (having to return to the hospital after being discharged – typically in less than 30 days)
  • Medication errors are PREVENTABLE and extremely important.

Tips for medication safety:

  • Take your medications as prescribed by your doctor. Know HOW to take them, WHEN to take them and WHY you are taking them.
  • Finish all of your medications.
  • Store your medications properly.
  • Keep a detailed record of all medications that you are taking (both prescribed and over the counter).
  • Read prescriptions and labels to make sure instructions are clear. If you have any doubts, consult with your pharmacist.
  • NEVER take any medications that are not prescribed for you.
  • DO NOT take expired prescriptions.
  • Dispose of any unused or expired prescriptions carefully: http://www.fda.gov/
  • Keep medications out of reach from children and pets.
